Apply Health Card Online

Online Application
  • If you have already secured an Official Receipt(OR)    Proceed to application
Online application is open from 09:00 AM until 4:00 PM but will only accept a maximum of 300 applicants per day.
Payment
If OR is not yet secured, you can Pay either Walk-In or Online through LandBank.

1. Walk-In
  • Go to City Hall Business-One-Stop-Shop (BOSS).
  • Go to teller assigned for Health Card and ask for requirements and amount of payment.
  • Go to cashier and pay.

2. Online Payment through LandBank
  • Proceed to payment
  • After completion of online payment, please wait for the Official receipt from the City Government of Tacloban that will be sent through email.
Check Status
You can check the status of your online application using the Application ID that was given to you after you submit your online application and with the mobile no. you use during application.
